THURSDAY, SEPTEMBER 9TH
5:00 - 7:00 PM

 


Your child will not be assigned a homeroom until these documents are returned. Every rising 1st-5th grade student must bring in the following items:

1. Current mortgage statement/current lease (if it has changed in any manner)

2. Current utility/Legal document

3. Car Tag Registration (must get an affidavit if you do not own a car)

What if the home or apartment is not in my name or I live with someone?

1. Two of the following:

-Utility Bill
-Car Tag Registration
-License
-Government Mail (DHS, Child Support Order)

2. Affidavit of Residency- must be signed and notarized by homeowner/renter
