Registration/enrollment packets are available for pick up all summer in the main office. The forms need to be completed and returned during registration times.

(Registration will continue beyond July 31; however, times may vary. To ensure a schedule is ready to go on the first day of class, it is recommended that you register by July 31.) Please have all information completed and bring the required information with you when you come in to enroll:

Forms needed for registration (bring with you when you come):

• Mississippi Certificate of Compliance Form 121
  
  This is the student's shot record recorded on a Mississippi form.  You may obtain this form by taking your student's shot record to the Desoto County Health Department, which is located on Highway 51, south of Hernando Square on the west side of the road.  Shots are given on Tuesday, Wednesday, and Thursday.  Telephone: 662-429-9814     Hours: 8-3, M-F  
  

• Certified Birth Certificate
  
  This form should include parents' names, city, state, and county of birth, along with date of birth. 

• Social Security Card 

• Last Report Card

• Official Withdrawal Form from last school  

• Transcript (may be unofficial copy) from previous school(s) including any credits earned toward graduation from grades 8-12

• Proofs of Residency
  
  Proofs must include one of each of the following:
  
  1.  Filed Homestead Exemption Application, mortgage
      documents, property deed, apartment or home lease (any of
      these documents presented must have physical address
       included)
                                           
  2.  A current utility bill:  light, gas, or water ONLY.
