2011-2012 School Registration

For all students currently enrolled at Lewisburg High School:

July 20th A-L 7am to 4pm.
(Last name beginning with the letter A-L)

July 21st M-Z 7am to 4pm.
(Last name beginning with the letter M-Z)

Failure to complete the requirements for registration will result in your student not being issued a schedule or assigned a teacher.

We encourage you to bring your documentation on the days listed above.

First Day of School is August 4, 2011.

ENROLLMENT REQUIREMENTS:

To enroll a child in school, parents must present school officials with the following:

  1. Print out the following information and fill it out: REGISTRATION FORMS 

  2. Certified copy of the child's birth certificate (not the mother's copy).  The County Health Dept. has forms for ordering this document.

  3. Shot record on a Mississippi Compliance Form 121 (From Mississippi health department.)

  4. Social Security Card

  5. Withdrawal or final report card from previous school.

  6. MINIMUM OF Three (3) documents for proof of residency in the name of parent(s) or  guardian(s) from the following list:

First Proof:

  • Mortgage Documents or property deed with name and address showing

OR

  • Apartment or home lease

Second Proof:

  • ONE Utility bill dated WITHIN THE PAST 2 MONTHS or deposit receipt ( water, gas,  electric --- NO CELL PHONES OR CABLE BILLS)

Third Proof:

  • Car Tag Registration in current address
  • Driver's Liscense in current address

 

Note:  Student must be living with person who has legal custody (verified by birth certificate, divorce decree, or court documents awarding guardianship).
