You must print each one of the forms that are listed below.  Complete each form and bring the completed forms and required residency information to the guidance office to register your child.  All forms must be completed and submitted at the time you register your child.

(ONLY COMPLETE PACKETS ACCEPTED)
PICK UP A PACKET IN MAIN OFFICE 8-2 M-F OR PRINT THE FORMS BELOW.
