Registration Information

Welcome Parents and Guardians

The DeSoto County School System provides an educational opportunity to students who reside in DeSoto County with their natural parents or a court-appointed guardian. 

In order to make registering your child more efficient, we will provide you with the necessary information you will need.  Please come by the guidance office and pick up the forms.  Due to the high volume of students that register we ask that you take the forms home and complete them.  Bring all the required documents at time of enrollment.  Registration times are as follows:  Monday – Thursday, 8-11:00am.  Please call to make an appointment if you are unavailable during these times.

We welcome you to Olive Branch High School and thank you for taking the time to prepare before you arrive to register your child. 

The following forms are necessary at the time of enrollment in a DeSoto County School:

    1. Withdrawal Form / Final Report Card
    2. Mississippi Health Form #121 showing two (2) measles vaccinations
    3. Two Proofs of Residence: A.) Mortgage document or
    lease
                                AND
    B.) Current utility: gas, water, or electric
    4. Certified Birth Certificate
    5. Social Security Card
    6. Mississippi Driver’s License

Parents should download the following informational and record forms from this web site.  Click the Registration Information link to see the following files.  (1)  Principal letter to the parents; (2) Home Language Survey; (3) Request for Records; (4) Permission to Photo; (5) Residency Registration Checklist; (6) Testing Statement of Assurance; (7) State Attendance Law; (8) Registration Form; (9) Student Information Form; (10) Graduation Requirements  Bring the completed forms along with the above information to the guidance office when your register your child. 

Additional Registration Information

·        If you are providing care while the parent or legal guardian is unable; approval for enrollment may be granted by contacting Mike Smith at the Board of Education.

·        If you would like to request a transfer into the Olive Branch School District, please contact Hank Babin at the Board of Education.

·        If you are residing outside of the Olive Branch District until you have completed a purchase within the district, please contact Hank Babin to request a special enrollment. 

·        If you do not have your own residence but are residing with a family within the Olive Branch District you must have an affidavit completed and notarized. You will be required to attend a Residency Committee Meeting.
