Residency Requirements for
2010-2011 School Year Registration
 
 

  • Mortgage/lease/rental agreement
  • Mississippi driver's license at stated address
  • Current utility bill dated June 2010 or later reflecting stated address
  • Current Desoto County auto registration reflecting stated address

Note: Students returning to the same school they attended last year (2009-2010) do NOT need to provide Mississippi driver's license and mortgage/lease/rental agreement unless changes were made and if copies are already in the student's cumulative folder.

The school will not accept incomplete packets of residency proofs. If proofs of residency are not available, an affidavit must be completed, notarized and submitted with separate alternative proofs from a government agency for both utilities and automobile registration.

Students who have not completed residency requirements prior to the start of the 2010-2011 school year will not be given a class schedule or be assigned a teacher.

 Tiffany Mooneyham is a graduate of Bartlett High School in Bartlett, TN.  She earned her Bachelor of Science degree in Education from The University of Memphis and is currently pursuing her Masters degree in School Counseling (M.Ed.) from The University of Mississippi. Ms. Mooneyham taught math for 7 ½ years at Olive Branch Middle School and has also worked as the coach/sponsor for both the cheer and dance teams at OBMS.
  Greg Jones is a graduate of McNairy Central High School. He earned his Bachelor of Arts degree in Church Music from Blue Mountain College. He earned the Master of Education degree in School Counseling from the University of Mississippi. Mr. Jones taught Special Education for 8½ years and has been a counselor for 13 years.


Enrollment Policy - Only the custodial parent/guardian may enroll a student in Olive Branch Middle School. The parent/guardian must present the custodial papers
at the time of registration. A copy of these papers will be kept on file in the student's records.
  • Proofs of Residency: A minimum of 2 documents in the name of the parent(s)
    or guardian(s) from the following list: mortgage documents or property deed, apartment or home lease (no handwritten or personal notes accepted), one
    utility bill dated within the past 2 months or deposit receipt on water, gas, electric.
  • Mississippi Certificate of Immunization Compliance Form: Mississippi Compliance Form 121, commonly called the "blue form," may be obtained from the local area health department upon receipt of the student's shot record. If a student is coming from another Mississippi school, the requirement for the immunization form will be waived until receipt of the student's school record
    from the previous school. If the original form is not in the student's record, another immunization form will be required.
  • Birth Certificate: A certified copy of the student's birth certificate must be presented to the school within thirty (30) calendar days from enrollment if the student is enrolling in a Mississippi school for the first time or if the certificate has not been recorded and verified on the record.
  • Social Security Number: The student's social security card must be presented so that the number can be recorded.
  • Last Report Card: A copy of the student's last report card or withdrawal from the previous school is needed.
  • Payment of Fees: All fees for classes such as band, physical education, where applicable, must be paid when a student registers for the class.
  • School Registration Form: A school registration form must be completed and returned to the counselor.
  • Request for School Records Form: This form must be completed so that the student's school records from his/her previous school can be requested. The name of the previous school and the address are required as well as the parent's signature, which indicates that the school records can be released to the student's new school.

Withdrawal Procedures -
To enroll a student in another school, you must first withdraw from the previous school. This is a state law.
  1. The legal guardian must come in, send a letter, or phone the counselor if a student is withdrawing from OBMS.
  2. All textbooks and library books must be retuned the day of withdrawal.
  3. It is the legal guardian's responsibility to clear all fines and account for all
    books.
  4. The librarian will sign the withdrawal form to clear the student from any
    library fines.
  5. The book clerk will sign off and clear the student of all book fines and give
    the student their copy of the withdrawal form.

Grades will be sent upon request of records. If report cards have gone out, those grades will be posted. Current grades will be sent upon request. This is our policy.

We DO NOT INTERRUPT classes to get exit grades.


 Mississippi Compulsory Attendance Law (Section 37-13-91)

Mississippi law requires that all children who have attained the age of six but have not attained the age of 17 on or before September 1, must attend chool regularly. PARENTS/GUARDIANS are responsible for making sure that their child(ren) attend(s) school. If parents refuse to comply with the law, the PARENT/GUARDIAN may be taken before the court and fined and/or jailed for EDUCATIONAL NEGLECT of their child. The penalty for this unlawful act up up to one year in jail or $1,000 fine or both. At 12 unexcused absences, Mississippi's compulsory school law is in violation.
