Enrollment Requirements
ENROLLMENT REQUIREMENTS FOR GRADES K-5
STATE LAW REQUIREMENTS: Kindergarten students must be 5 years old on or before September 1, 2012 First Grade must be 6 years old on or before September 1, 2012
MISSISSIPPI HEALTH FORM 121:
*If a student has previously enrolled in a Mississippi school, he/she will be admitted under the condition that the original health form be received at PHES within 30 days of enrollment. (PHES will request this form from previous school.)
**For students who have never enrolled in a Mississippi school, take student's shot record to:
Health Department, 6569 Cockrum, Olive Branch, MS 662-895-3090
Health Department, 2705 Hwy 51 S, Hernando, MS 662-429-9814
**If a shot record is not available, the Health Department will issue a temporary form that is good for 30 days. PHES will accept this form for enrollment. After 30 days, a permanent form will be required. ***Most Mississippi and some TN Doctors' Offices can also provide this service.
CERTIFIED BIRTH CERTIFICATE *Embossed with state seal. We do not accept Mother's Copy or Hospital Certificate. To obtain a birth certificate, write or call the Bureau of Vital Records in the state of birth. www.vitalchek.com SOCIAL SECURITY CARD THREE PROOFS OF RESIDENCY

CURRENT REPORT CARD OR WITHDRAWAL FORM
only for those entering grade 1-5
*Must have name and address of school.
