Requirements for Registration

Registration for New Students
 
Registration will not be accepted unless ALL information is provided:

 
4 proofs of residency
   1.   current lease or mortage (no hand-written letters /leases accepted) at stated address
   2.   current utility bill (gas or electric) at stated address
   3.   Desoto County car tag receipt at stated address
   4.   Mississippi Drivers License at stated address
 
Certificate of Immunization (Must be on a Mississippi 121 form, which can be attained from the Health Dept.)
 
Certified Birth Certificate
 
Social Security Card
 
Last report card/Withdrawal papers/Transcripts from previous school
 
**If you are living with someone and cannot provide proof of residency in your name...
    1.  You must have a notarized affidavit filled out by the person you are living with and they must provide the 3 proofs of residency in their name.
    2.  You must then have your driver's license, car tags, voter registration, or government mail showingyour name at the address where you are living.
  
 
 
Returning Student Registration           
ALL students MUST bring in 4 NEW UPDATED proofs of residency and fill out an updated data sheet.
